Admissions

Each resident at Covenant Nursing Home must be admitted by a licensed physician and remain under a physician’s supervision.

The facility reserves the right to evaluate all potential admissions on a case-to-case basis. This will be based on the residents’ needs and the staff’s ability to meet those needs. Covenant Nursing Home does not discriminate on the basis of race, ethnicity, disability, sex or veteran status.

  • Application for admission
  • Level I Screening for Pre-Admission and Annual Resident Review (PASSAR). This must be dated within 30 days prior to admission to our facility and completed by a Louisiana physician.
  • Current PPD/TB Test or Chest X-Ray (must be completed within 30 days of admission)
  • Complete list of medications ordered by a physician
  • List of medical diagnoses and/or complications substantiating the need for admission to our facility

Dining & Meal Service

One of the very best features of life at Covenant Nursing Home is the food service available each day in our Dining Room. We provide therapeutic and mechanically altered diets depending on the physician’s order. While it is not always possible to meet the individual preference of every resident at Covenant Home, we pride ourselves on our food service.

Life Enrichment

We offer a full range of activities and activity programs to try and meet the needs of all our residents. A monthly activity calendar is posted in each room while larger calendars can be viewed around the facility. Scheduled activities are primarily group activities but we also offer a wide variety of individual activities.

For individual enjoyment, the following items are always available upon request from the Activities Department: iPod shuffle and headphones/speaker, portable DVD player, puzzles, decks of cards, puzzle books, baby dolls, arts & crafts, board games, etc.

We have an on-site Beauty Shop and a licensed beautician is on the premises most Tuesdays. The facilities of the Beauty Shop are available to all residents.  See beauty shop price list here.

Nursing

Our Nursing Department is here to provide you with compassionate, professional care and treatment related to the delivery of medications, dressing and treatment of wounds, monitoring of any known physical conditions, reporting changes in progress of physical functioning to physician, monitoring and treating any pain and providing a host of other services related to care and treatment as ordered by physician.

Our Certified Nursing Assistants (CNAs) are available to assist with Activities of Daily Living such as eating, dressing, bathing, grooming and toileting. Our CNAs are happy to serve and provide any necessary assistance during the stay.

Resident Services

Our Resident Services department is here to assist with discharge planning, clothing needs, Medicaid eligibility questions and applications, and other needs such as dental, hearing, vision and podiatry.
